RESPONSIBILITIES:
The Health and Housing Coordinator’s responsibility is to oversee the SOAR program and Street Outreach by setting plans, procedures, and activities that will assist in the overall attainment of the Program’s goals. Responsible for setting up and providing all needed services to the participants. To assist at-risk of homelessness and homeless individuals and families in stabilizing their housing situations and assisting in the access of stabilization resources. To assist in the application, reconsideration and appealing at judicial level, processes of applying for SSI/SSDI. Coordination of health and housing services related to incarceration and street outreach to the homeless.
EXAMPLES OF DUTIES:
• Represent participants with medical professionals, Social Security Administration, Disability Determination Services, and court officials.
• Cultivate and nurture relationships, assuring that new or revised policies, plans, and procedures are
consistent with the Agency’s mission and values.
• Develop Street Outreach needs to support staff throughout our service area.
• Develop individualized plans for achieving greater participant self-sufficiency, housing stability, and
independence from public assistance including goals, strategies, and time frames.
• Provide crisis intervention & mental health de-escalation to youth/families when necessary.
• Act as a liaison between participants, County workers, medical professionals, attorneys, Disability
Determination Services, and Social Security Administration to problem-solve application process obstacles.
• Assure individualized plans and/or contracts are completed thoroughly, and that attainable and measurable
goals and objectives and strategies are signed by the participant and forwarded as needed.
• Complete thorough assessment summaries, including participants’ points of view.
• Ensure all HIPPA requirements are met when collecting, submitting, and holding medical records.
EDUCATION AND EXPERIENCE:
• Four (4) year degree in a mental health related field or have a social work license and 2 years’ experience or
a combination of six (6) years education and/or mental health work experience.
• SOAR Training Certificate or willingness to obtain within 90 days of employment.

• Ability to aid individuals and families in crisis situations.
• Ability to facilitate comprehensive assessments with participants.
• Ability to set, monitor, and evaluate participant goals.
• Ability to work independently and as part of a team.
• Demonstrated effective interpersonal, written, and verbal communication skills.
• Demonstrated ability to work with other services providers to advocate on behalf of participants for
improved accessibility to services.
• Demonstrated problem-solving and conflict resolution skills.
• Ability to travel regularly within the Nine (9) county service area and occasionally outside the service area.
• Demonstrated computer skills.
